Unusually high bifurcation of the median nerve resulting in two separate cylindrical compartments within the transverse carpal ligament: a case report

چکیده
Introduction The median nerve courses through the carpal tunnel before entering the hand. Numerous descriptions of this pathway exist [1, 2], which state that the terminal branches of the median nerve bifurcate into radial and ulnar divisions, at the distal border of the transverse carpal ligament. Several variants exist and have been documented. Some variations describe the course of the thenar branch, the accessory branches at the distal carpal tunnel, high division of the median nerve and accessory branches proximal to the carpal tunnel [3, 4]. We report a case of an unusually high bifurcation of the median nerve into radial and ulnar branches proximal to the transverse carpal ligament. Another factor that makes our case unique is that both the branches were encapsulated by fibers arising from the transverse carpal ligament and were separated into two separate compartments within this ligament.

A Comparison of Hand Pain and Hand Function after Z-plasty Reconstruction of the Transverse Carpal Ligament with Traditional Median Neurolysis in Carpal Tunnel Syndrome
Background: Carpal tunnel syndrome is the most common focal mono-neuropathy. A study was designed to compare the effects of traditional open carpal tunnel release with median neurolysis and Z-plasty reconstruction of the transverse carpal ligament on post-operative hand pain and hand function in patients with idiopathic carpal tunnel syndrome. Methods: Fifty-two patients with idiopathi...
متن کاملCarpal Tunnel Syndrome in Association with Klippel-Trénaunay Syndrome: A Case Report
Klippel-Trénaunay syndrome is a rare congenital condition that rarely affects peripheral nerves. Median nerve involvement at carpal tunnel level has only been reported in a few occasions in the medial literature. A 61 years old Caucasian female patient with Klippel-Trénaunay syndrome presented with a 10 months history of paraesthesiae and numbness affecting the median nerve distribution area...
متن کاملCase Report: A Case Report on Variation in Bifurcation of Brachial Artery
Variations in arterial anatomy are less frequent, contrary to the venous system, and most of these variations affect visceral arteries. Variations in the brachial artery are the most frequently reported and so far a minimum of six different patterns have been described. The most common of these patterns is the superficial brachial artery, which lies superficially to the medi...
متن کاملCarpal tunnel syndrome due to lipofibromatous hamartoma of the median nerve.
This report describes a rare case of secondary carpal tunnel syndrome due to a lipofibromatous hamartoma of the median nerve. Excision of the fibrofatty tissue between the nerve fascicles without risking damage to the fascicles was impossible. The transverse carpal ligament was incised and an epineurotomy was performed. Within six months, the 25-year-old female patient's symptoms were much impr...
متن کاملCarpal tunnel syndrome caused by lipoma: a case report
Lipoma is a relatively frequent, benign soft-tissue tumor rarely located in the hand. A lipoma of the hand causing a carpal tunnel syndrome by compression of the median nerve is exceptional. We report the case of a 70-year-old female presenting with a carpal tunnel syndrome. A compression of the median nerve by a lipoma was discovered during surgery. Transverse carpal ligament release with lipo...
